Lucknow: A green corridor was created to carry a harvested organ of a man killed in a road accident for a patient suffering from critical organ failure here, officials said on Wednesday.Chief Executive Officer and Managing Director of Apollomedics Hospital Dr Mayank Somani said, "A 21-year-old man severely injured in a road accident was admitted to our hospital. He passed away due to a traumatic brain injury. Following consultation and with the consent of the family members of the deceased, it was decided to harvest his organs for transplant." He said two kidneys, liver and the corneas of the deceased were harvested for transplant in different patients.To carry one of the harvested kidneys from Apollomedics Hospital to Sanjay Gandhi Postgraduate Institute of Medical Sciences (SGPGIMS), some 11 kilometers away, a special green corridor was set up. Deputy Commissioner of Police (Traffic), Lucknow, Subhash Shakya told PTI that with the help of the green corridor, the harvested organ was taken to SGPGI within seven minutes. Green corridor refers to a special traffic route set up to carry organs for transplant, critical medicines or patients to save human life.According to SGPGI sources, the kidney was transplanted into a 35-year-old woman suffering from kidney failure. The other kidney and the liver were transplanted to different patients in Apollomedics Hospital. New Delhi: Delhi Police have busted a kidney transplant racket and arrested 10 persons in connection with the case, an official said on Wednesday. The accused have been identified as Kuldeep Ray Vishwakarma (kingpin), Sarvjeet Jailwal (37), Shailesh Patel (23), Md Latif (24), Vikas (24), Ranjit Gupta (43), Dr Sonu Rohilla (37), Dr Sourabh Mittal (37), Om Prakash Sharma (48) and Manoj Tiwari (36).It is learnt that the accused persons illegally transplanted more than 20 kidneys.Deputy Commissioner of Police (south), Benita Mary Jaiker, said an information was received on May 26 at the Hauz Khas police station regarding an illegal kidney transplant racket operating in the Hauz Khas area, which targeted poor people and induced them to sell their kidneys.A tip-off was received that a person will be taken to a private lab for pre-anaesthesia check-up by the members of the racket and subsequently a trap was laid near the lab. "One person was spotted who informed that he was being taken to the lab by Sarvjeet and Vipin on the pretext of treatment for his abdomen pain, but when he realised that they were taking him there for kidney donation, he had arguments with them following which they went away," the DCP said. Accordingly, the police registered a case under the relevant sections of the Indian Penal Code and the Transplantation of Human Organs Act and took up the investigation. The police constituted a team which conducted raid at A Block, DDA Flats, Paschim Vihar, where four persons, including accused Shailesh Patel and three others -- Diwarkar Sarkar, Ashwini Pandey and Rizwan -- were found present. On questioning, it was revealed that these three persons were taken there for illegal kidney transplantation."The three persons were about to be taken for kidney transplant, but due to the police's timely intervention, the plan was foiled," the DCP said.Both Sarvjeet and Shailesh were then questioned at length and on the basis of sufficient evidence, they were arrested.On interrogation, the duo revealed that they used to target poor people for kidney and brought them to Vikas and one doctor who gave them Rs 30,000-40,000 as commission. They further sold the kidneys to rich persons at higher rates. Vikas provided accommodation to all the sellers at Paschim Vihar and further sent them to Sonu Rohilla through accused Ranjeet Gupta at Gohana, Sonipat, for illegal kidney transplantation. The police team along with a forensic squad then conducted raid at a hospital in Gohana where a setup was created by the accused persons for illegal kidney transplantation and arrested Sonu Rohilla. Another accused, Dr Sourabh Mittal, an anesthesist who worked with a reputed hospital in Delhi, helped perform the illegal transplants, the officer said.During interrogation, it was revealed that Kuldeep Ray Vishwakarma was the main mastermind behind the racket who convinced others for illegal transplants. Kuldeep also selected the clinic of Sonu Rohilla for execution of the crime. He was the person who paid all the other accused persons. The gang had performed 12-14 illegal transplants during the last 6-7 months at the Gohana setup. The accused used to target young boys in the age group of 20 to 30 years, who were in desperate need of money.—IANS

New York: According to a new study, a new algorithm can analyse thousands of variants across the genome and estimate a person's risk of developing chronic kidney disease.The study was published in the journal, 'Nature Medicine'."With this polygenic method, we can identify individuals at risk decades before the onset of kidney disease, and those with high risk might adopt protective lifestyle changes to reduce that risk," said Krzysztof Kiryluk, MD, associate professor of medicine and a physician-scientist in the division of nephrology at Columbia University Vagelos College of Physicians and Surgeons.Early detection of kidney disease could prevent many cases of kidney failure and reduce the need for transplant or dialysis, but the disease is often silent until it has caused significant kidney damage.Genetic testing could offer a way to predict a person's risk of kidney disease well before symptoms appear, but thousands of inherited variants are likely involved and most have only small effects. Adding to the complexity, certain genetic variants are more common in some ethnicities than others."In most populations, we can't just look at one or two genetic variants and tell you what your risk is," said Kiryluk. "Thousands of variants are likely contributing," she added.Kiryluk and his team described their method and tested it on 15 different groups of people, including those of European, African, Asian, and LatinX descent. The algorithm analyzes variants of a gene called APOL1--known to be a common cause of kidney disease in people of African descent--and thousands of other kidney disease variants found in people of all ancestries. Across all ancestries, people with the highest scores (in the top 2 per cent) had triple the risk of kidney disease as the general population, equivalent to having a family history of kidney disease.The study also confirmed that APOL1 was an important risk factor in people of African descent. But even when APOL1 is present in an individual, other genes can increase or decrease the risk of developing chronic kidney disease."For people of African ancestry, APOL1 is an important part of the picture, but not the only part," Kiryluk said. This information may be significant when new drugs being developed specifically for people with APOL1 become available. "Individuals with APOL1 but low polygenic risk may not need specific interventions, since their risk could be comparable to that of the general population," Kiryluk said. "In contrast, individuals with the highest genetic risk--those with APOL1 and a high polygenic risk--may benefit the most from lifestyle changes or drug treatment," she added.More testing of the new prediction method is needed before it can be used in clinical settings, Kiryluk adds.The method is being tested in a large national study, called eMERGE-IV, that will screen participants and offer additional follow-up and lab testing for people at high genetic risk. New Delhi: Doctors have performed a challenging 'Auto-Kidney Transplant' on a young patient, who had the history of a stone in left ureter, the pipe connecting the kidney and urinary bladder at a private hospital in the capital city.The department of Urology &amp; Kidney Transplant of Sir Ganga Ram Hospital last month received a 29-year-old patient from Punjab who had a history of stones in the left ureter. A local Doctor from Punjab tried to remove the stone but during the process, 25-26cm of left ureter also came out along with the stone, said the hospital on Tuesday. "In a normal patient, there is one kidney on the left and one on the right side and two ureters connecting these kidneys to the bladder. But in this case, we were surprised to see the left kidney lying alone without any connection with the bladder," said Dr. Vipin Tyagi, Senior Consultant, who operated the patient. "Since the patient was young and the intestine is not the ideal substitute for the ureter reconstruction. We decided to perform 'Auto-Kidney Transplant', which means in this patient taking the normal kidney out from the left side and bringing it close to the bladder on the right side and connecting it with blood vessels going from the abdomen to the right leg (External Iliac Vessels). Now both the kidneys are on the right side," he added further.Dr Tyagi further added: "The kidney was close to the bladder but with the gap of 4-5cms. So, we decided to reconstruct a tube of 4-5cms using the wall of the urinary bladder. As soon as this reconstructed tube was connected to the bladder, the blood flow to this kidney restarted and immediately urine started coming out through this tube." "The options before us were either to remove the kidney or remake the missing connection between kidney and bladder by using intestine or perform kidney autotransplant," Dr. Sudhir Chadha, Co-Chairperson, Department of Urology, said. Ahmedabad: A team of doctors led by senior surgical oncologist Dr. Nitin Singhal, has set the record for surgically removing the “heaviest kidney tumor” in a child and has been recognised by the India Book of Records (IBR).Notably, the team successfully removed a large Wilms’ tumor weighing 3.1 kg from the left kidney of a 3-year-old child, providing a new lease of life to her.Following the surgery, a claim was registered with the IBR. It was thoroughly scrutinised by a team from the IBR after which a post-attempt felicitation and final approval have been provided. Commenting on the development, Dr. Singhal said, “It was a very challenging surgery for us, but thankfully, we could successfully execute it. After one-and-a-half-year of the surgery, the child is living a normal life like any other kid.”As per the case details, the child was brought to hospital when her parents noticed a huge increase in the size of her belly over a period of 2-3 weeks. On consulting senior pediatrician Dr. Pushkar Srivastava, they were referred to consultant pediatric oncologist Dr Hemant Meghani.Dr. Meghani investigated the child and found a rare condition in which a huge mass was arising from the horseshoe kidney. The mass was pushing the diaphragm, causing the child to have severe breathing difficulties. The case was then discussed with the Multidisciplinary Tumor Board. In normal circumstances, such huge masses are generally diagnosed by a biopsy and treated by chemotherapy respectively. But since there were several risks with this approach, emergency surgery was decided in this case as an exception.The four-hour long surgery was performed by Dr. Singhal and his team which included anesthetist Dr. Ankit Chauhan and pediatric surgeon Dr. Kirti Prajapati.Dr. Singhal said, “This case highlights the lack of awareness in our society in relation to paediatric tumours which have a good prognosis if treated properly and on time. In fact, it is interesting to know that such tumours in this age group have much better survival chances than adult tumours.” The surgery was done at Apollo hospital, Ahmedabad. —IANS

Washington: A new study found that taking the amino acid lysine, as an over-the-counter dietary supplement, protects laboratory animals from kidney injury.
The study, Associate Professor Markus Rinschen from Aarhus Institute of Advanced Studies and Department of Biomedicine, was primarily conducted on rats with high blood pressure - hypertension - and associated kidney disease. But a small pilot study confirms that the amino acid could have a similar effect in humans, without definite evidence for clinical effects on kidney disease.
"We discovered that there is an accelerated transformation of the amino acid lysine in humans and animals with kidney disease. And the study shows that the intake of lysine protects the kidneys and prevents hypertensive kidney disease rather effectively, at least in animal models" explains Markus Rinschen. Side effects still unknown
It is estimated that up to 10 per cent of the adult population suffers from chronic kidney disease, although often mild and without symptoms. The most frequent causes of kidney disease and kidney failure are diabetes or high blood pressure, and the consequence is a much higher risk of heart attack or stroke.
However, Markus Rinschen assesses that he will not be able to start treating patients in the clinic for at least five years, and he stresses that it is too soon for people with kidney diseases to run out and buy lysine tablets.
"We don't know the side effects or the underlying mechanisms yet, and human metabolism is much more complex than a rat's metabolism," he says. "We need to conduct more research into animal models because we have not yet clarified the dominant mechanism behind the result. We found three different mechanisms, but we don't know whether one, two, or a combination of all three, is the decisive factor."
In the long term, the result will be particularly interesting to health researchers, doctors, nephrologists, physiologists, endocrinologists and nutritionists.
"It would be great if kidney patients could achieve results by changing their diet," says Markus Rinschen.
"We want to understand kidney metabolism, and this is a big step. Giving patients a substance they already have in their body and creating clinical results would be a new and surprising discovery," says the researcher, who hopes that the study can lead to a more general understanding of beneficial metabolites.
"The study shows how dynamic and unexplored our metabolism still is, and that we need holistic approaches to understand it. Diet, metabolism, heart and cardiovascular system - many things contribute to the development of kidney disease." —ANI

Metabolism Might Be Vital For Future Kidney Disease Treatment: Study

Augusta (Georgia): A large group of bacteria present in our soil, water, and showerheads are generally harmless to most people, but a recent study suggested that they are linked to an elevated risk of death in people whose kidneys have failed.
In what appears to be the first study of its kind, investigators at the Medical College of Georgia and Charlie Norwood VA Medical Center in Augusta looked in the United States Renal Data System at patients with end-stage renal disease, or ESRD, who also had a diagnosis of infection with the nontuberculous mycobacteria, or NTM, group.
They found a significant and independent increase in mortality with an NTM diagnosis in these patients, indicating that early diagnosis and treatment of an NTM infection may improve survival in ESRD patients, they report in the Journal of Investigative Medicine.
"It's important to be alert that certain patients can be at higher risk for NTM and that NTM carries a risk for mortality," said Stephanie L. Baer, MD, MCG infectious disease physician and chief of Infection Control and epidemiology at the Charlie Norwood VA Medical Center.
These "opportunistic" pathogens, which have even been found in dialysis machines, tend to only cause serious problems when a patient has a compromised lung or immune system function.
Patients whose kidneys have failed are considered to have a compromised immune function and generally considered at higher risk of infection, so investigators at MCG and the VA wanted to better identify prevalence, risk factors as well as associated deaths in those who also had an NTM infection.
They looked specifically at the 0.3 per cent of the 1.1 million patients in the database with ESRD and an NTM diagnosis over a decade that ended in 2015. Patients were either on dialysis or had a kidney transplant.
"We looked for risk factors for the bacterium ... and we looked at the different diseases it causes, like lung disease, skin disease, and disseminated disease and looked at the mortality of those patients," says Baer, corresponding author. With a few exceptions like skin disease, NTM infection pretty much always increased mortality in patients with ESRD, she said. The investigators emphasize that their findings highlight the need for physicians to remain vigilant for NTM infections in ESRD patients.
"This is showing an association," said coauthor Wendy B. Bollag, PhD, cell physiologist in the MCG Department of Physiology. "We don't know if NTM directly causes the mortality or is more of a red flag to their physician to treat that patient aggressively in that moment."
It does mean patients with ESRD may need testing for NTM, if there are symptoms that indicate it may be present, and treatment with the appropriate antibiotics, they say. It may additionally indicate they need to be screened for conditions like infection with HIV, which directly targets the immune system, and those who have received a kidney transplant may need adjustments in the medications they must take to keep their immune system from attacking the transplanted organ.
To help ensure that they were looking specifically at the association between ESRD and NTM, the investigators controlled for other known risk factors for NTM infection such as being Black, having diabetes or liver disease, or being HIV positive. The database did not contain information on other potentially confounding factors like BMI and blood levels of proinflammatory factors. They found a higher risk of NTM infection with peritoneal dialysis versus hemodialysis. Peritoneal dialysis can be done at home and uses a cleansing fluid and the lining of the abdomen to filter waste products, compared to machine-based hemodialysis, but exactly why there was a difference was not clear. Rheumatologic disease like arthritis, which may require medicine that suppresses the immune response and so painful inflammation, also increased the NTM risk as did a history of organ transplant, and the investigators note that due diligence in needed in individuals with these added risk factors. They did note that patients with kidney transplants who had NTM infections were less likely to die, potentially because of factors like restored kidney function, closer follow-up with health care providers and that patients selected for transplant may be more generally healthy than those on dialysis.
The database did not specify which NTM was most prevalent, but they suspect it was the M. avium complex, which is by far the most common type in the Southeastern United States, Baer said.
"It is all around us. It can cause an allergy called a hot tub cough or sneeze," Baer said.
There are more than 70 identified species of NTM and growing. These bacteria wrap themselves in a protective biofilm and are becoming increasingly disinfectant and antibiotic-resistant. NTM infection can produce nonspecific symptoms like fever, weight loss, night sweats and lethargy. Lung nodules and lymph nodes that don't resolve can be telltale signs. "The good news is it's all around us and most of us are never bothered by it, until our immune system becomes compromised," Bollag noted. ESRD and NTM infections are both on the increase, with rates of ESRD more than tripling between 1990 and 2018, according to the Centers for Disease Control and Prevention. The increase in NTM likely is due to an aging population with COPD and an increasing number of individuals with a compromised immune response, Baer said.
According to the National Institute of Diabetes and Digestive and Kidney Diseases nearly 786,000 people in the United States have ESRD, 71 per cent are on dialysis and 29 per cent had a kidney transplant. Mycobacteria are a large group of organisms known to cause serious conditions like leprosy and tuberculosis. NTMs are part of that group, minus those that cause leprosy and tuberculosis. —ANI

Oklahoma: Every year, over 8,000 individuals die while waiting for a kidney transplant, many of whom have been on donor waitlists for four or more years, hoping for a miracle. Researchers revealed that these deaths occur as a result of a worldwide scarcity of kidneys for transplantation, currently, there are no effective methods for determining the viability of enough donor kidneys donors to fulfill demand.
A team of researchers from the University of Oklahoma and the OU Health Sciences Center, with assistance from LifeShare of Oklahoma, as well as researchers and clinicians from the University of Massachusetts Amherst, Worchester Polytechnic Institute and Georgetown Medical Center will collaborate to investigate the use of optical coherence tomography to evaluate donor kidneys and develop new scanning methods and machine learning algorithms to reduce the evaluation time of donor kidneys while substantially increasing the information about the viability of these organs for transplant surgeons and clinicians.
"Our goal is to develop a better, more objective method for evaluating kidney quality than is offered by current methods," Tang said. "We want to get more kidneys in the hands of transplant surgeons and make better use of the donor kidney pool."
The current process for screening donor kidneys uses two methods - pathological scores based on anatomical features from a biopsy and the Kidney Donor Profile Index (KDPI) derived from the donor's medical history. However, clinical research indicates that those current methods have limited discriminative power.
"By scanning the whole kidney surface, our device intends to eliminate the uncertainty created by the biopsy/KDPI paradigm," Tang said.
In addition to Tang, the OU research team includes Chongle Pan, Ph.D., associate professor of computer science and microbiology, Kar-Ming A. Fung, M.D., Ph.D., professor of neuropathology and surgical pathology, director of neuropathology and director of electron microscopy at OU HSC; and Zhongxin Yu, M.D., a board-certified pathologist with OUHSC.
When assessing donor organs for transplant, Tang said there are two golden standards, pre-transplant biopsy score results and post-transplant clinical outcomes. Tang and his team believe that optical coherence tomography, or OCT, scanning technology can aid in predicting both. OCT is a non-invasive imaging test that uses light waves to take cross-section pictures of tissue, providing what is effectively an 'optical ultrasound' by imaging reflections from within tissue to provide cross-sectional images. The key benefits of OCT are the acquisition of live, sub-surface images at near-microscopic resolution with instant, direct imaging of tissue morphology without the need for preparation of or contact with a tissue sample.
"What we want to do is to automate the evaluation process," Pan said, "so the data coming from the OCT device can be directly analyzed by machine learning algorithms and output a score that is comparable to the score surgeons are currently using. The surgeon can look at this score and determine if this kidney is right for their patient."
Currently, pathologists often use a biopsy to assess kidney health which only provides information about the specific spot the biopsy is taken from. Since OCT is non-invasive, it is possible to scan the whole kidney and get a more complete view of the organ's overall functional capacity.
"A kidney is a three-dimensional object. (Whereas) a biopsy can only assess one specific spot in that kidney to determine transplant viability, OCT scans can image about 1-2 millimeters below the surface of biological tissue," Tang said. "For transplanting kidneys, this is very good because it is the most representative area of kidney functionality. By scanning the whole organ instead of taking a biopsy from a single area, the transplant surgeon can assess the spatial distribution of the function of the kidney and decide if it is a good match for their patient."
OU researchers will be responsible for acquiring, scanning and obtaining an initial histological analysis of the kidneys, as well as developing OCT scanning and machine learning components. LifeShare of Oklahoma will provide 200 donor kidneys over the next four years. These kidneys will be scanned by Tang's team and OU pathologists will provide the histology results.
Kidneys deemed to be viable for transplant will be added to the United Network for Organ Sharing system, the only organ procurement and transplantation network in the U.S.
The research team will observe the clinical outcomes of patients receiving these kidneys post-transplant, and OCT images of the kidneys will be correlated with post-transplant renal function to establish imaging algorithms and guidelines for OCT imaging of kidneys prior to their transplant.
For kidneys deemed non-viable by current pathological standards, the project will combine the biopsy results with OCT images to build a database and correlate the OCT image with the histology result, with the goal of verifying that the OCT images align with the histology results from the biopsies.
This combined pool of data will allow the team to develop deep-learning-based image processing algorithms to quantitatively assess parameters observed in OCT scans as indicators of the functional status of kidneys. As part of the project, they will also develop a robot-assisted automatic 3D scanning OCT device to image human kidneys prior to their transplantation. Pan explained the diagnostic criteria developed for machine learning during this project will be based on the same scoring sheet clinicians are already using, except the sheet would be reproduced using OCT data rather than pathology slides. "Right now, 40 per cent of donor kidneys are discarded as being unfit for transplant," he said. "By our estimate, of that 40 per cent, there are a substantial amount that are actually usable. We are hoping to decrease the number of discarded kidneys to 25 per cent and save more people's lives." —ANI

Researchers Suggest New Scanning Methods To Reduce Evaluation Time Of Donor Kidneys

Hyderabad (The Hawk): A 53-year-old man's life was saved by surgeons at the Asian Institute of Nephrology and Urology (AINU) in Hyderabad who were able to successfully remove a kidney tumour the size of a football and weighing 10 kg.
The hospital announced on Thursday that this was only the second successful surgery to ever be recorded in the nation and the first in the Telugu States.
Dr. Taif Bendigeri and Dr. Rajesh K. Reddy were part of a team of urologists led by Dr. Mallikarjuna C that completed this difficult procedure.
The patient, a Kadapa resident, was referred to AINU with abdominal swelling, according to medical professionals.
Doctors examined the patient and discovered a significant abdominal mass lesion. The tumor's origin in the left kidney was identified through imaging.
A large portion of the abdominal cavity was taken up by the mass, which also forced the intestines into the right lower quadrant.
"Given the size of the tumour, we decided against using a robotic procedure and went with open surgery. With a lot of work, the tumour could be successfully removed. Following surgery, we found the tumour to be enormous—it was the size of a football. Dr. Mallikarjuna explained that a microscopic examination revealed the tumour to be a cancerous growth (renal cell carcinoma).
Although the patient was in pain, it is surprising that she did not pay much attention to the swelling in her abdomen. The left kidney that had cancer was removed by our team. The tumour was completely removed according to the clear surgical margins at the microscopic level. Since the tumour had not spread to any other organs, it was discovered that the patient did not require any additional treatment. He has been warned not to disregard follow-up. Drs. Rajesh K. Reddy and Taif Bendigeri concurred, saying that this would aid in routine monitoring.

Patna (The Hawk): According to his son and the deputy chief minister of Bihar, Tejashwi Yadav, doctors in Singapore successfully transplanted the kidney of the former chief minister of Bihar, Lalu Prasad Yadav, on Monday.
For the transplant, Lalu Prasad was checked into the Mount Elizabeth hospital in Singapore. The kidney was given by Rohini Acharya, his daughter.
Tejashwi published a video from his official Twitter account after the successful transplant, stating that his sister Rohini and father Lalu Prasad were in good health.
"After a successful kidney transplant, my father and sister were moved from the operating room to the intensive care unit. They both interacted with us and were in good health. Everyone's blessings are appreciated, Tejashwi said.
The second-eldest daughter of Lalu Prasad and Tejashwi's sister, presently resides in Singapore with her husband and kids. Rohini made the decision to donate her kidney after Lalu Prasad had a severe illness in both of his kidneys.
Singaporean medical professionals examined both the donor and the receiver and approved the transplant. Since the donor and the beneficiary were members of the same family, the doctors saw no complications.
At many locations around Bihar, Rashtriya Janata Dal (RJD) supporters organised a "havan."
Tej Pratap Yadav, Lalu Prasad's eldest son, also hosted a "havan" in his Patna home. RJD MLC Qari Suhaib sold a "chadar" in the Patna High Court's "Mazaar."
Lalu Prasad had a severe illness in both of his kidneys and was incarcerated at Ranchi's Hotwar jail for the fodder scam. Dr. Vidyapati was the first to recommend that he get a kidney transplant after he was admitted to the Ranchi Institute of Medical Sciences in Ranchi. Later, he was also granted admission to AIIMS, Delhi.
According to state RJD President Jagadanand Singh, "Rohini Acharya has given them the message that no one is bigger than mother and father in today's world where children of big families do not care for their fathers and moms."
"Lalu Prasad is a leader of Dalits and a member of the weaker class in the country," he continued. Everyone, even the leaders of the opposition parties, is praying for his recovery.

Hyderabad (The Hawk): At the KIMS hospital in Hyderabad, medical professionals performed a liver and kidney transplant on a 50-year-old patient for the first time in India. Her liver weighed 12 kg.
A group of reputable surgeons, including three liver transplant surgeons and a kidney transplant specialist, carried out simultaneous liver and kidney transplantations.
The surgery was carried out in the first week of November, the hospital revealed on Thursday.
Usha Agarwal, a homemaker from Siliguri in West Bengal, is the patient whose life was saved and who has been returned to normalcy.
Doctors claimed that her liver was so colossal that it took up her entire abdomen and displaced her intestines. A healthy liver weighs little more than 1.5 kg and is located in the right upper quadrant of the abdomen.
She had a hernia, a big liver, ascites in her abdomen, and it was difficult for her to move. In 2019, when she first felt heavy, she was encouraged to get a transplant.
Cysts (fluid-filled cavities) arise in the kidneys and liver as a result of gene abnormalities in polycystic liver and kidney disease, according to Dr. Ravichand Siddachari, Consultant and Chief of Liver Transplant and HPB Surgery. No symptoms appear in the patients till they are in their 30s. They start to experience the symptoms as the cysts enlarge. They have the potential to get extremely large, and the resultant buildup of water in the belly can cause breathing issues and hernias. As their kidney functions deteriorate, they might require dialysis. All of these symptoms were present in this patient, with the exception of a large, ruptured hernia.
"Unsurprisingly, given that the liver had taken up the entire abdomen, it was one of the trickiest procedures to perform. Detaching the liver from its connections while preserving the vital organs needed for transplantation was a difficult feat. But we were able to successfully transplant a fresh liver. Dr. Uma Maheshwara Rao, Consultant Urologist and Renal transplant surgeon, revealed that the replacement kidney was implanted through the same cut after making a pouch in the belly (usually, another cut is required for a kidney transplant).
The fact that the patient had healed well and had been released from the hospital pleased the doctors who had carried out two uncommon transplantations on that person on the same day.
It was one of the most fulfilling operations because it not only saved the patient's life but also enabled her to finally be free of all physical and emotional pain and suffering. Without any issues, she can start carrying out ordinary tasks, they stated.

Edinburgh (Scotland): A severe illness that can cause the kidneys to cease working unexpectedly could be managed with existing medications, according to a new study.
In a study in mice, scientists found that medicines usually used to treat angina and high blood pressure prevented much of the long-term damage to the kidney and cardiovascular system caused by acute kidney injury (AKI).
The study is published in Science Translational Medicine. It was funded by the Medical Research Council and the British Heart Foundation.
Experts hope the findings will pave the way for improved treatment of AKI - a common illness that occurs in approximately 20 per cent of emergency hospital admissions in the UK.
The condition is usually caused by other illnesses that reduce blood flow to the kidney, or due to toxicity arising from some medicines.
AKI must be treated quickly to prevent death. Even if the kidneys recover, AKI can cause long lasting damage to the kidneys and the cardiovascular system. Of those who survive an episode of AKI, 30 per cent are left with chronic kidney disease (CKD). The remaining 70 per cent that recover full kidney function are at an almost 30-fold increased risk of developing CKD.
A team from the University of Edinburgh found that patients with AKI had increased blood levels of endothelin - a protein that activates inflammation and causes blood vessels to constrict. Endothelin levels remained high long after kidney function had recovered.
After finding the same increase in endothelin in mice with AKI, experts treated the animals with medicines that block the endothelin system. The medicines - normally used to treat angina and high blood pressure - work by stopping the production of endothelin or by shutting off endothelin receptors in cells. The mice were monitored over a four-week period after AKI. Those that were treated with the endothelin-blocking medicines had lower blood pressure, less inflammation and reduced scarring in the kidney. Their blood vessels were more relaxed and kidney function was also improved, compared with untreated mice.
Dr Bean Dhaun, Senior Clinical Lecturer and Honorary Consultant Nephrologist at the University of Edinburgh's Centre for Cardiovascular Science, said: "AKI is a harmful condition, particularly in older people and even with recovery it can have a long-term impact on a person's health. Our study shows that blocking the endothelin system prevents the long-term damage of AKI in mice. As these medicines are already available for use in humans, I hope that we can move quickly to seeing if the same beneficial effects are seen in our patients."
Professor James Leiper, Associate Medical Director at the British Heart Foundation, said: "Impaired kidney function that results from acute kidney injury can also increase a person's chance of developing and dying from heart and circulatory diseases, so it's vital we find ways to reduce this risk. "This promising research suggests that widely available medicines could help to tackle the impact of acute kidney injury before it can cause damage and further complications. While further studies will be needed to demonstrate whether this treatment is safe and effective for patients, this early research is an encouraging first step." —ANI

Philadelphia: Chronic kidney disease (CKD) in children is rare, which makes research difficult and leaves large gaps in the quantity and quality of information that is useful for pediatric patients with the condition. The largest prospective studies also have limited usefulness, as they are constrained by selection bias and relatively small sample sizes for different causes and courses of the disease.
In a new study led by Children's Hospital of Philadelphia (CHOP) and Nemours Children's Health, researchers have overcome the obstacle of scale by analyzing electronic health record data from PEDSnet, a national multicenter pediatric network, to identify a large cohort of children with CKD, evaluate CKD progression, and examine clinical risk factors for kidney function decline. The findings were published in the Clinical Journal of the American Society of Nephrology.
"The overarching goal of this project was to demonstrate that real world EHR data can be used to model kidney function decline in children. Chronic kidney disease in children is rare and as such there are few high-quality studies that inform clinical decision making," said Caroline Gluck, MD, physician, Division of Nephrology at Nemours Children's Health, Delaware, adding, "In contrast to prospective cohort studies, this study represents an unbiased sample of the US source population and to our knowledge represents the largest cohort of children with CKD to date."
To better understand factors contributing to the decline in kidney function in patients with CKD, the researchers focused on children from six pediatric health systems in the PEDSnet database who were seen between January 1, 2009, to February 28, 2022. Of more than 7.1 million children, the researchers identified 11,240 (0.157%) with CKD. They divided the group into subcohorts based on CKD type: glomerular, non-glomerular, and malignancy-associated. CKD progression was determined based on estimated glomerular filtration rate (eGFR), a calculated index of kidney function based on serum creatinine level that determines the stage of kidney disease; the need for long-term dialysis; and the need for a kidney transplant.
The researchers found that the following attributes in children with CKD were associated with a more rapid decline in kidney function: disease of glomerular or malignancy-associated origins; high levels of protein in the urine (proteinuria); hypertension; younger age; more advanced CKD; male sex; and greater medical complexity at the start of follow-up care. For example, over an average follow-up time of five years, 40% of patients with glomerular CKD required long-term dialysis or transplant or experienced more than a 50% reduction of eGFR, whereas the same was true of only 13% of patients with non-glomerular CKD.
Given the size of the population analyzed in the study - which also included children with cancer, who have a high burden of CKD but have been excluded from or underrepresented in prior studies -- the findings can be applied more broadly to the pediatric CKD population.
"Children with the risk factors identified in our study are of particular concern for CKD progression and may be targeted for comparative effectiveness studies to preserve kidney function," said senior author Michelle Denburg, MD, MSCE, a nephrologist at Children's Hospital of Philadelphia and co-director of the CHOP Pediatric Center of Excellence in Nephrology and of the Penn-CHOP Kidney Innovation Center.
"The findings and methods in this study are foundational to future research in children with CKD, including the PRESERVE study I am co-leading with Dr. Christopher Forrest, Director of PEDSnet, which is leveraging EHR data from 16 pediatric health systems to analyze blood pressure management and preservation of kidney function in pediatric CKD. This study can also serve as a roadmap for use of EHR data networks to adequately power the study of rare disease," she said. —ANI

Bengaluru: The Manipal Hospitals of Bengaluru hosted the second annual bean-shaped (kidney) Cyclothon 2.0 on Sunday to raise awareness about the need of maintaining good kidney health.
The Cyclothon was broken up into two loops totaling 100 km, and was finished in 2.5 hours. The first loop comprised 47 km, going from Old Airport Road to Millers Road, Malleshwaram, Yeshwanthpur, and Hebbal before returning to Old Airport Road.
The second route was a 53-kilometer circuit that began at Old Airport Road and went through Whitefield, Varthur Road, Sarjapur, Jayanagar, and back to Old Airport Road.
Almost 600 people took part in the cyclothon, including doctors, 10 celebrities, and 23 influencers who all helped to spread the word about the importance of kidney health, early detection and prevention, and improved access to renal care.
Some well-known actors and actresses from the Sandalwood film industry came up in favour of the campaign, including Praveen Tej, Manvita Kamath, Vikky, Arun, Nagabhusana, Sonu Gowda, Vasuki Vaibhav, Shwetha Shrivastav, Kushee Ravi, and Amrutha Iyengar.
Hospital Directors from all around Bengaluru joined Dr. H. Sudarshan Ballal, Chairman of Manipal Hospitals, for the official start of the event at Manipal Hospital Old Airport Road.

Hyderabad: A 45-year-old male patient had 154 stones removed from his right kidney by urologists at the Asian Institute of Nephrology &amp; Urology (AINU), Secunderabad.
A CT scan was performed on a patient from Telangana's Ramagundam district, and the results showed several calculi in the kidney, the largest of which measured about 62 millimetres by 39 millimetres.
All of the kidney's calyces have been invaded by these enormous calculi. His assessment revealed that his blood sugar levels were out of control.
Percutaneous nephrolithotomy (endoscopic kidney surgery) was performed once his blood sugar levels were stabilised.
Multiple pieces of the 62 mm x 39 mm large stone were retrieved.
Secondary stones were successfully recovered without any damage. A total of 154 stones, including the largest one, were taken out.
Speaking about the patient, Raghavendra Kulkarni, Consultant Urologist, AINU, said: "Staghorn calculi are complex renal stones that occupy the majority of the renal collecting system. These stones are associated with high morbidity, which can be in the form of recurrent urinary tract infections, kidney injury and neglected / untreated cases can end up with need for dialysis. Managing patients with staghorn calculi can be challenging, especially in diabetes mellitus patients. Advances in technology and techniques have enabled effective treatment with minimal morbidity.
"The patient underwent multi tract PCNL to achieve complete stone clearance. The largest stone measuring about 62mm x 39mm was fragmented and retrieved in multiple pieces. All the secondary stones from all the calyces were retrieved intact. Secondary stones numbered approximately 153 and the size of the stones varied from as small as 2-3 mm to approximately 10mm. The blood loss was very minimal with no transfusions, and the patient recovered well from Anaesthesia," Kulkarni added.
For patients with complicated or significant renal stone loads, PCNL has become the treatment of choice.
The effectiveness and safety of PCNL in treating renal stones has been confirmed by numerous research.
Having diabetes mellitus raises the risk of postoperative complications, notably infections.

They have been charged with violations of Code of Criminal Procedure sections 307, 326, and 420 in India. It's conceivable that more suspects will be apprehended.
Prameswara Rao, a doctor at a private hospital, along with middlemen Kamaraju, Srinu, Shekhar, Elina, and Kondamma, were arrested on Sunday, according to police.

AIIMS Rishikesh Becomes The First Govt. Hospital To Perform Kidney Transplant

London: The use of anabolic steroids can cause serious side effects, including heart failure and depression, for years after the end of your use, a new study showed on Saturday.

According to the studies presented at the European Congress of Endocrinology in Istanbul, anabolic steroids (synthetic hormones that mimic the naturally-occurring sex hormone testosterone), are used to increase muscle mass and boost athletic performance.

These performance-enhancing drugs have negative side effects, such as breast growth, hair loss, decreased testosterone levels, erectile dysfunction, and an increased risk of heart disease, stroke, and liver or kidney failure in men.

In the first study, the researchers examined 64 healthy men, between the ages of 18 and 50, who did recreational strength training in Denmark.

Of these men, 28 were using anabolic steroids, 22 were former steroid users, and 14 had never used these steroids.

The researchers used a Positron Emission Tomography-Computed Tomography (PET-CT) scan with the radioactive tracer Rubidium-82 to assess how much blood flowed to their heart muscle while resting and exercising and discovered that both former and current users had poor blood flow to the heart.

The findings indicate that former steroid users are more likely to develop heart disease when compared to those who have never used them, said the study.

"Previous studies have shown that the heart function almost normalises after anabolic steroids are discontinued, but our study suggests that former anabolic steroid users are at an increased risk of heart disease years after stopping as cardiac microcirculation -- the blood flow through the smallest vessels in the circulatory system -- seems persistently impaired," said lead author Dr Yeliz Bulut.

Moreover, Dr Bulut and colleagues conducted another study in which they collected questionnaires and blood samples from three groups of men aged 18-50 years: 89 current anabolic steroid users, 61 former steroid users, and 30 men who had never used steroids before.

The study found that former anabolic steroid users report a worse quality of life on their physical and mental health, such as fatigue, social functioning and emotional well-being, despite stopping years earlier.

"Our study adds to the growing body of literature that an impaired quality of life in previous anabolic steroid users seems to persist years following cessation and could be caused by both withdrawal and/or hypogonadal symptoms due to a sudden drop in testosterone levels in the blood," said Dr Bulut. —IANS

Washington: Blood pressure is frequently measured in hospitalised patients. Severe hypertension"&gt;hypertension can result in a heart attack, stroke, or damage to blood vessels and organs such the heart, brain, kidneys, and eyes. The majority of hospitalised patients, on the other hand, have transiently elevated blood pressure without signs of organ damage, which is known as silent hypertension"&gt;hypertension and is infrequently treated with blood pressure medications. However, there is little data to guide such treatment decisions. In a retrospective cohort study, Beth Israel Deaconess Medical Centre (BIDMC) physician-researchers looked at the management of high blood pressure in almost 66,000 older persons who were hospitalised for non-cardiac reasons. The researchers discovered that having extensive antihypertensive therapy as an inpatient was associated with a higher risk of adverse events, especially for individuals taking the medicine intravenously rather than orally. The findings, published in JAMA Internal Medicine, do not support treating asymptomatic raised blood pressure in hospitalised older persons and emphasise the need for more research into best practises for inpatient blood pressure management. "While the benefits of lowering chronically elevated blood pressure in the outpatient setting are clearly defined and include reductions in mortality and cardiovascular events, better evidence is needed to inform clinical decision-making regarding inpatient blood pressure management," said corresponding author Timothy S. Anderson, MD, MAS, a clinical investigator in the Division of General Medicine at BIDMC. "In the hospital, blood pressure is often elevated due to pain, fever, anxiety, new medication and other hospital factors. It is not clear that treating transient elevations with blood pressure medications is helpful, it may instead result in overtreatment." Using clinical and pharmacy data from the national Veterans Health Administration (VHA), Anderson and colleagues compared outcomes of hospitalized patients with elevated blood pressure who received intensive blood pressure treatment in the first 48 hours after admission to those who did not. The primary outcome was a composite of adverse effects including inpatient mortality, acute kidney injury, cardiac injury, stroke, and transfer to the intensive care unit. The cohort included 66,140 older adults, primarily male, who were hospitalized for non-cardiac reasons and had elevated blood pressures in the first 48 hours of hospitalization. One in five patients (or more than 14,000 patients) received intensive treatment for blood pressure, defined as additional antihypertensive medications the patient had not been taking at home prior to hospitalization. Of this group, 18 percent (or more than 2,500 patients) received antihypertensive medication intravenously. Compared to hospitalized patients with elevated blood pressure who did not receive intensive treatment within the first 48 hours of hospitalization, patients who received antihypertensive medication were at greater risk for adverse clinical outcomes, including cardiac injury, acute kidney injury, and ICU transfer. Receiving antihypertensives intravenously further heightened the risk. "These findings suggest that the common practice of acutely treating asymptomatic inpatient blood pressure could be harmful and the use of intravenous antihypertensives in particular should be discouraged," said Anderson, who is also an assistant professor of medicine at Harvard Medical School. "Until we have more definitive randomized clinical trial data, our findings suggest that the safest path forward is likely to rethink the underlying reason for inpatient blood pressure measurement and reorient clinical practice. In combination, these findings suggest that pharmacologic treatment of asymptomatic elevated inpatient blood pressure should be the exception rather than the rule." —ANI
